ESPANHOL Administración y salud (I) El personal directivo no tiene una idea definida de cuál es su función y qué se espera de su trabajo; en consecuencia, tiene como idea central el deseo de ascender en la escala jerárquica sin capacitarse, centrado sólo en la imagen y el prestigio, o en la autoridad, ya que es el modelo que ha tenido durante mucho tiempo, por lo que el resultado es siempre el mismo: duplicación de actividades, incapacidad de satisfacer las necesidades y conciencia de haber obtenido sólo resultados modestos; al mismo tiempo, se dan cuenta que son inseguros al tomar decisiones administrativas, las cuales sólo las efectúan por intuición o por crisis, lo que los lleva a la frustración. Al respecto existe una controversia sobre quién debe administrar los servicios de salud. Paradójicamente se afirma que el médico no está calificado para administrar, aunque se dice que es conveniente que un médico administre establecimientos de salud porque se ganará más fácilmente la confianza, el respeto y la cooperación de todo el personal, por lo que estará en mejores condiciones de tomar decisiones. Por otra parte, se dice que los profesionales no médicos con preparación específica en administración de la atención de la salud tienen destacadas actuaciones al tener a su cargo hospitales y clínicas, situación común a muchos países. Es innegable que una persona que puede conciliar ambas posturas e intereses es el médico-administrador, con preparación específica en administración, ya que la razón de ser de las instituciones que conforman el sistema de salud es precisamente el servicio a la salud, por lo que las decisiones importantes deben ser tomadas por médicos. Se hace cada vez más evidente la necesidad de que la función directiva en las organizaciones de atención a la salud no sea asignada a quienes no cumplan con la formación correspondiente en administración en salud, sino que la selección del personal directivo se lleve a cabo de forma adecuada por competencias y no bajo el esquema de amistad y compadrazgo, aunque el candidato demuestre sentido común y deseos de asumir el puesto, pero desconozca lo más elemental para ejercer estas posiciones de manera profesional. Cuando se administran los recursos de una institución de salud, no puede hacerse de forma intuitiva; la responsabilidad moral y legal es muy grande y para desempeñarla bien es necesario prepararse.
